Form Catalog

The form and waiver catalog makes setting up forms a breeze. Copy example waivers, client intake forms, client feedback and more from our form catalog.

Form catalog will only be visible to businesses with an active Form & Waiver add-on subscription.

How to use the form catalog

  1. Navigate to Forms & Waivers -> Form Catalog.
  2. Click on a category of forms & waivers.
  3. Click on a form name you’d like copied to your account. Make sure you carefully read the description of the form before copying to your account.
  4. Rename your form, if required
  5. Click Add to my account

Preview and edit before sharing

The catalog form items are intended to provide a foundation for your form, but may require editing. Before sharing any form or waiver copied from the form catalog, please follow the steps to edit and preview the form.

  1. Navigate to your form list
  2. Click on the form you recently copied from the form catalog
  3. Click edit located on the left side of the form’s detail page
  4. Edit the “form settings” to meet your needs
  5. Edit all form fields to meet your needs (pay close attention to any logo place holder or reference to business name you need to replace).
  6. Preview the form within the form edit dialog. You may also want to save your edits, copy the sharable shortened form link, and walk through the steps a client would to complete this form. Make sure you do NOT click “submit” while testing/previewing your form, otherwise the form will no longer be editable.
